List of Docs to Bring

Failure to bring all required documentation with you to your appointment will delay your assistance. We require below documents:

Child

• Child's Birth Certificate or Confirmation of Birth from the Hospital
• Child's Current Immunization Certificate (Form 3231)
• Social Security Cards/Number for all household members
• Parent or Legal guardian Photo I.D.
• Child's Health Insurance Card (only if available)
• Proof of Family Income for previous 12 months. Example proofs can be (All W-2s, Income Tax documentation such as 1040, 1040A, Pay Stubs (12 months worth), Child Support (court order or previous 12 months payment statement), TANF award letter(s), SSI award letter(s)
• IEP or IFSP documentation if applicable

Expectant Mother

• Expectant Mother's Birth Certificate or Confirmation of Birth from the Hospital
• Expectant Mother Proof of Pregnancy
• Expectant Mother's Health Insurance Card
• Social Security Cards for all household members
• Parent or Legal guardian Photo I.D.
• Expectant Mother Health Insurance Card (only if available)
• Proof of Family Income for previous 12 months. Example proofs can be (All W-2s, Income Tax documentation such as 1040, 1040A etc, Pay Stubs (12 months worth), Child Support (court order or previous 12 months payment statement), TANF award letter(s), SSI award letter(s)
